Annual rewards – Once the below activities are completed and member completes the attestation for each:

- Annual Preventative Visit - $50.00
Diabetic rewards – The plan determines if the member is eligible for these tests and if they are deemed eligible, it will show on the members portal. Membership attests to these tests once each activity is completed, and each member will receive the rewards below.
- Diabetes - Eye $50.00 for retinal eye exam
- Diabetes - Glucose $25.00 for A1C test
- Diabetes - Kidney $25.00 for kidney health testing
